About MPI Premiums
As a result of Collective Bargaining, effective January 1, 2013, the Motion Picture Industry (MPI) Active Health Plan requires that Participants with eligible dependents pay a premium to maintain their dependents’ coverage (medical, dental, vision, prescription drugs, and behavioral health benefits). This change affects how Participants and dependents manage their healthcare.

30 to 60 days before premiums are due, you will receive regular Premium Notices that detail your outstanding balance and payment due date.

Forms
Reinstatement Request Form - Use this form to request a reinstatement of Active Health Plan coverage for yourself and/or your dependents.

Premium Refund Request Form - Use this form to request a refund of your health care premium balance.

(Note: Forms are valid only when both your MPID and signature are provided.)

    PREMIUM CREDIT/REFUND POLICY

    UNUSED BALANCE/ACCOUNT CREDIT
    Unused premium payments to MPI that result in a balance will be credited to the Participant’s premium account for future use and maintained as a credit balance for up to 2 years.

    The Participant may apply any credit balance to future premium payment(s). If the Participant has not re-qualified for coverage within a 2-year period, a refund will be issued automatically to the Participant.

    REFUNDS
    Credit refunds can be requested by the Participant at any time and will be issued by check within 30 days after MPI’s receipt of the Participant’s written request:
