PREMIUM CREDIT/REFUND POLICY

UNUSED BALANCE/ACCOUNT CREDIT
Unused premium payments to MPI that result in a balance will be credited to the
Participant’s premium account for future use and maintained as a credit balance
for up to 2 years.
The Participant may apply any credit balance to future
premium payment(s). If the Participant has not re-qualified for coverage within
a 2-year period, a refund will be issued automatically to the Participant.
REFUNDS
Credit refunds can be requested by the Participant at any time and will be
issued by check within 30 days after MPI’s receipt of the Participant’s written
request:
